Maybe they do not have a birthday set, so the system is setting a date for them ?

Yeah, I cheaply fixed it by adding a "if more than 20 show up in a day, assume the bug is there,"
I've since added a "don't count birthdays if the user hasn't logged in for a while", so the number of birthdays is now greatly reduced, and the old bug's popped up again! D'oh!
