In Sweden, we have a bank called SEB ("Skandinaviska Enskilda Banken"), with the final B meaning "bank." I hate it when people say the SEB bank, much like your FPS shooter example.

how's about ATM Machine (Automated Teller Machine Machine)?

A lot of bad grammar doesn't bother me, because i end up using "y'all" a lot. "ain't" doesn't seem to bother me either. The main thing that bothers me is when people use words incorrectly because they don't understand their meaning, most commonly i see it with "irony" and "ignorant". What's ironic about that is how ignorant people are for using "irony" and "ignorant" incorrectly. heh.

What bugs me is when people say is, instead of are. Like, "who is you?". I mean, how much harder is it to say are, instead of is? Where did these people even go to school? If people used correct grammar maybe they'd be easier to understand. But... oh well.

Pronouncing words wrong isn't really grammar but "ask" should be pronounced "ar-ss-k" or, if you insist on being northern, "ah-ss-k" not "ar-x"

In written form, mis-use of apostrophes and incorrect there/their/they're and your/you're (and Yaw I guess ) and and "etc." being written "ect"

Generally though I'm with Jay on the "Should have..." issue. It's come from lazy speech and too much contraction to "Should've" "Might've" "Would've" etc. (Interestingly, out of those last three, FireFox's spellcheck only liked "Would've")

Oh and one that bugged me back in the BlitzCoder days was the star character * being called an asterix. No, Asterix is a french comic book character who had a friend called Obelix, the star is an asterisk. And while I'm on the subject, Asterix was the short one, Obelix was the big muscular one in blue and white trousers, shaped like an obelisk !!!

And starting a sentence with "And"... oops!

I always get the it's/its one wrong.

I find it helps if you remember that "his" doesn't have an apostrophe, either, while they both show ownership.

Down here where I live (the south, barely tho, North Carolina) they have a handful of phrases that really bother me down here. One of them is "Ya know"! If I KNEW I wouldnt be listening to you now. I would have stopped you a long time ago and saved you a breath or five. And another one "Aint that right!" I dunno. What the hell! You didnt even ask me if it was....oh well you should have asked me my opinion a long time ago! Now I'm just gonna tell you "NO" just to piss you off!

And another thing that bothers me is the misspronouncing of "CARAMEL" ITS Pronounced CARE-UH-MEL! Not CAR-MAL! I have a sister whose name is CARA and its pronounced CARE-UH not CAR-UH. THERE'S AN 'A' IN THERE PEOPLE!!! PRONOUNCE IT!

And dont get on the boat about how the southerners dont correctly ENUNCIATE their words. Thats a whole other day there!!!
